To request a copy or report of your imaging studies, please log in to the patient portal and click on "Request Records" and then select "Request Medical Records."
Patients can also fax a signed release form asking for their imaging records to 813-499-8582 or pick up a CD of the records in person at the Picture Archiving and Communication System (PACS) Library (located in room 13309, third floor, Moffitt Clinic Building, Magnolia campus.)
